The following forms (1-5) may be needed for road captains and are available for download as PDFs:

1) Release form for adults - Required:

a) for all participants 18 or over for OPEN events.

b) for all particpants 18 and over who have not signed a Chapter Enrollment and Release Form for CLOSED events.

2) Release form for minor - Required for all minors, under the age of 18, for all events, each time, there is no annual release form for minors.

3) Minor's Assumption of Risk - Required for all minors aged 12 or older but less than 18, IN ADDITION TO the release form for minor referenced in 2).
